　平成16年4月25日（日）東京八重洲ホールにて千葉大総合診療部主催、卒後・生涯医学臨床研修部共催による上記セミナーが開催された。千葉大関係者のみならず、医師、学生、出版関係者など関東近縁から定員を超える約６０名が参加した。
　前半は総合診療部長司会の元で、会場の前方に集められた学生を対象に台本なしの症例検討会が実演された。参加者はその様子を後ろから観察し、学生の意見を聞きながら自由に発言する形式である。外来で遭遇した２症例が呈示され、臨床推論を用いた病歴聴取でどこまで診断に迫れるかについて、和気藹々とした雰囲気の中で活発な討論が繰り広げられた。
　特別講演は、元アイオワ大学家庭医療科臨床助教授のラルフクヌッツン医師により、医療制度の変革が与えた米国プライマリケアへのインパクトについて衝撃的な話しが披露された。医療が市場原理によって動く米国では、株主利益を最優先する企業の論理により、良心的な医師がより安い賃金で雇える医師によって容赦なく置き換えられてしまうという事態にまで至っていることに驚かされた。彼自身、病院の経営者が変わっただけで、90日以内に給料25％カットを受け入れなければ解雇するという突然の通達を受け、昨年、彼を含めた家庭医グループは全員職を失ったという。クヌッツン医師は良医として地元ではつとに有名であり、かかりつけ医制度が定着している米国での家庭医大量解雇に地域住民は反対運動を起こしたが、結局、市場原理に押し切られたそうである。この解雇劇によって長年のかかりつけ医を失った住民の損失は計り知れない。わが国のメディアは米国医療のよい面だけを取り上げる傾向がある。しかし乳児死亡率などのWHOによる米国国民の健康水準は、わが国の２倍もの医療費を使いながら世界37位である（日本は世界第１位）。利潤を追求した結果として、軽症患者のみを保険に加入させる“サクランボ摘み”をはじめ、株主へ顔を向けた患者不在の医療を展開している米国医療の陰の部分をもっと知ってほしいと講演は締めくくられた。市場原理は一旦導入されてしまうと撤回困難である。わが国でも医療特区における株式会社参入の動きも起こっており考えさせられる内容であった。
　続いてナンシークヌッツン氏によって、自身の職業であるフィジシャンズアシスタント（PA）の現状と展望についての講演が行われた。わが国では馴染みのない制度であるが、医師の管理のもとで診察、検査、処方、お産や麻酔など医師と同等の仕事を行い、家庭医不足の米国でプライマリケアを担う重要な一翼となっているということであった。同様の職種にナースプラクティショナー（NP）があり、現場のニーズに合わせた研修を行うPAに対して、看護業務を医師の業務へ発展拡大させた制度であるとの説明があった。実際の仕事内容に大きな差はないという。
　よい意味でも悪い意味でもプライマリケア先進国アメリカで活躍されているお二方の現場の声は、今後わが国の医療が進むべき道を考察する上で大いに参考となった。
